What Are the Fees for Requesting Medical Records from UT Southwestern Medical Center?

Depending on the request type, there are different fees for getting your UTSW medical records.

Electronic Copy

$6.50 flat rate for the delivery of health records via a CD, electronic file portal, or MyChart Patient Portal. Any applicable processing and shipping fees may also apply per Texas state guidelines.

Note that electronic medical record request fees are waived for armed services members, veterans, and employees of UT Southwestern Medical Center.

Paper Copy

When requesting paper copies of your UTSW medical records, the cost depends on the number of pages plus any applicable processing and shipping fees.

  • 1-10 pages: $6.50
  • 11-20 pages: $25.00
  • Over 20 pages: $0.50 per additional page

Radiology Images

There is no charge for the first copy, but additional copies are $6.50 each. The radiology images are delivered via a CD, electronic file portal, or LifeImage. Per Texas state guidelines, any applicable processing and shipping fees may also apply.
