TMC Healthcare has several ways to request your medical records. This can include checking online with MyChart, ordering them by mail or fax, or calling the radiology department.

MyChart

Checking with MyChart is probably the easiest route to go because it's an electronic form of your medical records. Other medical facilities often use it, so you can sometimes view a complete version of your medical records and not just those offered by TMC Healthcare.

To get on MyChart, you first need to create a MyChart account. You'll need to request an activation code. This will require filling out a form that asks the following.

  1. First and last name
  2. Date of birth
  3. sex
  4. Medical record number
  5. Mobile phone
  6. Home phone
  7. Email address
  8. Social security number

Once you do this, you can go back to the Sign-Up page and enter your activation code. Next, you'll enter your zip code and date of birth. When you are signed up, you can chat with your doctor, access lab results faster than ever, request a prescription, and manage your appointments.

The best part is that you can print your medical records using MyChart, so you don't have to go through TMC Healthcare unless you just prefer to.

If you need a film or digital image, you'll have to call the Radiology Department. The number is (520) 324–5166. You'll also need to fill out a medical release form, which you can use the methods we've already mentioned to send it in.
