If you are a patient at Scripps Health, you can request your or send your request through mail, email, or fax. Once your request is received at Scripps Health, your medical records are processed within 15 business days and delivered to you through your preferred delivery method.

The process of submitting your request for medical records to Scripps Health is easy. However, it can be frustrating and inconvenient, especially if you have to fill out the forms yourself. Request Medical Records through Fax, Mail or Email

If you prefer this method or don't have a MyScripps account, you can follow these few steps to submit your request.

  1. Download the authorization form in English or Spanish.
  2. Fill out the form and sign. If you request radiology reports and images, ensure that you include the specific dates when the procedures were done.
  3. If you want to allow a representative to request the medical records on your behalf, you may need to fill out a Designation of Personal Representative
  4. form.
  5. Fax, mail, or email the signed forms the Scripps Health.

You will need to pay a certain fee for the medical records depending on the following calculation.

  • One-time $5 for each patient request
  • $0.02 per page charge for electronic records over 250 pages
  • $0.10 per page for non-electronic records over 50 pages
  • $6.50 for radiology images CD copy or Nucleus.io Imaging Cloud upload.

Medical records sent to another doctor do not attract any charges. However, if the records are requested from an insurance company or office, they attract a $15 processing fee.
