Accessing Your Records Electronically
Salem Hospital provides patients with access to their records through My Chart. To view your records on My Chart, you must first go to the website and sign up. After that, you can follow these steps:
- If you have requested that the documents be sent to your My Chart, you can log in and click on the Download Document to download your requested records.
- The records will be available on My Chart for 30 days, and you can print them.
- You can request additional records by clicking on Document Center. All records requested in the past 30 days will be available.

- You will receive an email containing a link that you must click to complete the process.
- Select "Open New Request"
- Choose "Request Type"
- Fill in the required fields and click "Submit Request."
