If your healthcare provider is requesting your records for you, they must follow a similar process.

  1. Make sure they send the request to the correct facility.
  2. Fill out the required form, including the medical record number, relationship to the patient, patient information, facility, medical provider name and address, requested information, a reason for the request, patient signature, and witness signature.
  3. Submit the request via mail, fax, or in-person.

If a healthcare provider needs information STAT, they should contact the Health Information Management Unit at the facility to get the records as soon as possible.

What Are the Fees for Requesting Medical Records From LAC + USC?

While a fee is not always required for medical records, some facilities will require it. At LAC + USC, there are a few fees you could be asked to pay when requesting medical records.

  • Up to $0.25 per page on copied paper.
  • Up to $0.50 per page for records copied from microfilm.
  • Up to $6.00 for records on a Compact Disc (CD).
  • A flat fee of $15 if the records are delivered to an attorney.

Copy fees can be waived in specific circumstances, such as if the records are to be used to help the patient prove their eligibility for a public benefit program. You may receive an invoice after receiving the records if payment is required, so be sure to watch out for one after requesting your records.
