DHR Health has two ways that you can request your medical records:

Your medical records are free to obtain unless you will be requesting your records and images on a CD. If that's the case, there may be a fee associated with your request, and DHR Health won't release your medical records until you've paid the fees in full.

If you're requesting medical records to be released to another healthcare system, there will be no fee.

Are There Any Restrictions On Requesting Medical Records?

Your medical records are your private information, and your healthcare provider is legally obligated to keep them safe. Additionally, DHR Health is legally required to release your medical records to you when they've been requested.

State and federal laws protect your privacy, and they ensure that your medical records aren't obtained by

  1. Anyone besides you,
  2. An authorized medical professional, or
  3. A person who you have authorized to request them.

In some cases, a subpoena can also allow medical records to be released to the courts.

Parents can request the medical records of their minor children under the age of 12. If your child is over 13, they're generally given the same protection as an adult.

If you want to , DHR Health is legally obligated to release them to you within 15 business days.

How to Transfer Your Medical Records to DHR Health

If you want to request your medical records from DHR Health, you need to call their phone number, which you’ll see below. They'll be able to tell you what forms need to be filled out, and where you need to submit the forms.
