What Medical Records Are Restricted From Being Released?

Print Form
- Go to this page and print it out.
- Fill out the page completely.
- Note that there is a section where you can denote whether or not you want your detailed medical information to be shared.
- Mail it to the local VA facility that you go to.
Online Form
- Go to this page and scroll down to the VA Blue Button heading.
- Click on the box to sign in.
- Once you are signed in, click on Health Records on your dashboard.
