How to Get a Salt Lake County Marriage Certificate on Your Own
You can apply for a marriage certificate online. You just need to fill out the form found here and visit them at the address below:

You can also request your marriage certificate by mail. Mail your request to the address given above. You should include the following information:
- Name of person requesting copy
- Address
- Names of both parties at the time of the application
- Date and place where the marriage took place
- License number if available
- $5 per certified copy, payable by check or money order to the Office of Sherrie Swensen, Salt Lake County Clerk
Lastly, you can also request your marriage certificate over the phone. Contact the County Clerk's Office at (385)468-7300. Requirements for requesting by phone are:
- You must be one of the spouses on the marriage license
- Provide the year and names as they appear on the certificate
- Provide a return phone number
- Provide a Visa or MasterCard with the following information:
- Name as printed on the card
- The credit card statement address
- Card number
- Code on the back of the card
- Expiration date
Once the information is collected the County Clerk's Office will process your request and send the marriage certificate to the address provided.
