How to Request Medical Records From MGH

MGH enables patients to request their medical records and any amendments made to their records by submitting a specific form to the Mass General Brigham Medical Records Department. MGH states that patients who are minors and patients unable to give consent will require a signature from the following:

  • A parent
  • Guardian

In addition to this, MGH may also require additional authorization for information that is specifically protected or information that is considered privileged.

Depending on the type of record you are requesting, it may take up to 30 days to receive them. With that said, there are three ways you can request medical records. Patients can request their  either online, by mail, or by fax.

The easiest and quickest way to request MHG medical records is online. The Patient Gateway is MGH's secure, online portal that houses patients' health information. Patients can do the following using the Patient Gateway:

  1. View their medical documents
  2. Download their medical documents
  3. Transfer documents between other medical facilities
  4. Print medical documents

If you don't have an account with MHG's Patient Gateway, you will need to sign up for an account. You can enroll in Patient Gateway by going to their website and clicking on "Enroll Now". Once you're logged on to your account, you will need to take the following steps:

  1. Go to "messaging". This should be located at the top bar.
  2. Click, "Request records".
  • Requesting Medical Records by Mail or Fax

To request medical records by mail or fax, you will need an authorization form from the medical facility where you are requesting records. There are 12 hospitals in which patients can request their medical records, including MGH. Additionally, patients can download authorization forms in 3 different languages, including English, Spanish, and Portuguese.

If you need medical records from multiple facilities and would like to request your entire medical records, MGH states that patients should use the Mass General Brigham/Partners HealthCare authorization form.

Getting access to medical records on your own can be a tedious process. Most hospitals require you to fill out a variety of forms and you often have to wait longer than desired to receive records. Not only that, but some medical records can be more difficult to get, which can result in a frustrating process.
